Admission
General Guidelines
Every student-applicant should accomplish the following guidelines:
- An applicant must complete the application procedure to be considered an official applicant.
- Student-applicant must fill up the student’s application form and submit in the Admission’s Office.
- College applicants must choose one college or diploma program. Senior High School applicants must choose one track and one strand.
- After accomplishing the pre-registration procedure, the student-applicant will be scheduled for an entry assessment in the form of a written and one-on-one interview.
- Student-applicant must submit the needed documents as enrollment requirements.
Requirements
Freshman:
- Application form including 2x2 ID photos
- Original form 138 (High School Report Card)
- Original Certificate of Good Moral Character (signed by the Principal)
- Photocopy of Authenticated Birth Certificate (PSA)
- NCAE result
- Marriage Contract (if married)
- Long brown envelope
Transferee:
- Application form including 2x2 ID photos
- Transcript of Records
- Original Certificate of Good Moral Character
- Honorable Dismissal or Transfer Credential
- Certificate of Eligibility to Transfer (if Honorable Dismissal/Transfer Credential is not yet available)
- Photocopy of PSA Authenticated Birth Certificate
- Marriage Contract (if married)
- Long brown envelope
